SHBG crashed on tirz — is that a problem?

Baseline SHBG was 42 nmol/L. After 14 weeks on tirz 10mg/week, -28 lbs, SHBG is 24.

Total T unchanged (684 -> 702). Free T is higher now of course (lower SHBG + same total). Feel fine.

Is the crashed SHBG bad on its own, or is it just 'better insulin sensitivity -> lower SHBG' which is the expected story?

SHBG CV is ~8-12% within person. 42 -> 24 is clearly outside noise so it's a real shift, but as cbc said, it's a predictable shift in a direction that usually means 'metabolism is healthier'.

Lower SHBG on improved insulin sensitivity is the expected story. You're seeing the metabolic improvement, not something bad.

That said, SHBG 24 is on the lower end. If you feel fine, Free T is fine, estradiol in range, no acne/irritability — you're good. The number by itself isn't worth acting on.

only thing id want to know is whether your e2 actually stayed in range or if it just feels fine. lower SHBG means higher free e2 too, not just free T, and some guys dont notice the mood/water retention sides until they look back at labs. did you get e2 tested pre and post or just going off feels?
