Tweaked the left achilles 4 months ago running hills. MRI shows tendinosis, no tear. Tried conservative rehab for 10 weeks with marginal results. Time to bring out the tools.

Goal: back to running 20mi/week by end of cycle without pain.

Baseline:

  • Single leg heel raise: 8 reps (right does 25+ without issue)
  • Pain on palpation: 6/10 at the insertion
  • Pain first steps in morning: 7/10 (classic tendinopathy sign)
  • Running volume right now: 0

Stack:

  • BPC-157 500mcg subq 2x/day
  • TB500 5mg/week subq, split into 2.5mg x 2
  • Running both for 8 weeks

Rehab: heavy slow resistance calf raises, 3x/week, per the Alfredson protocol modified.

Let's see.

Week 1: no real pain change. No injection site issues. Morning pain still 7/10.

Week 4: morning pain 2/10. Palpation pain 2/10. Single leg heel raise 18 reps. Starting some easy flat jogging at week 5 if this trajectory holds.

Don't chase miles. First run should be 1 mile max, super easy pace.

Week 5 first run: 1 mile on grass, easy pace. Zero pain during, very mild soreness after that resolved by morning. Morning pain in general now 1/10.

Week 6: running 2-3mi every other day. No pain, no setback. Heel raise matches the other side at 25+ reps.

Holding instead of pushing at the tail end is the right call. Most people lose the recovery by getting greedy.

Week 7: up to 12mi/week running. Started hill work again — did the same hill that originally hurt me and it felt bulletproof. Going to hold here through week 8 rather than push.

Week 8 wrap-up:

  • Morning pain: 7/10 → 0/10
  • Palpation: 6/10 → 0/10
  • Single leg heel raise: 8 → 27
  • Running volume: 0 → 12mi/week

Going to taper both compounds this week and reassess in a month. If it holds I'll update.

Four weeks post-cycle update: still zero pain. Running 18mi/week now, adding speed work. Holding.

Solid progression man, especially holding back instead of chasing the 20 when you hit 18. That's the move most people screw up. Curious if you're gonna blast it again or just cruise on maintenance work now that the tendon's holding?
