BPC-157 for rotator cuff — 12 week log (FINISHED, wrap-up inside)

Starting this to keep myself accountable. Tore something in my right rotator cuff back in October throwing a medicine ball (yes, stupid). MRI said partial supraspinatus tear and tendinopathy, no surgery recommended. PT for 8 weeks got me maybe 40% back. Still wakes me up at night if I sleep on it.
Goal: get pain-free sleep and back to pressing by week 12.
Starting stats:
- Pain at rest: 3/10
- Pain with overhead reach: 7/10
- Night pain waking me: 4-5 nights/week
- Overhead press: cannot currently do one with the empty bar without pain
- ROM: missing ~20 degrees of external rotation
Stack:
- BPC-157 500mcg subq 2x/day (morning + evening), bilateral abdomen
- Running 12 weeks, reassess at 4 and 8
Other variables:
- PT exercises 5x/week (the boring stuff — Ys, Ts, external rotations with band, scap pushups)
- Sleeping in a shoulder sling-ish setup to stop myself from rolling onto it
- No pressing of any kind during log
- Collagen 15g/day with vit C (probably placebo but whatever)
Will update weekly. If I see zero change by week 6 I'll reassess.

Week 1 update: no real change in pain yet, which is expected. Injections have been painless, no site reactions beyond a little redness that fades in an hour. Sleeping marginally better but honestly that's probably just the sling preventing me from rolling over.

How's the ROM feeling? Sometimes that moves before pain does in my experience.

ROM is the same but pain at end range is way down, interestingly. I can get further before it starts to complain, even though the actual stop point hasn't moved.

Following. Subq for shoulder is what I'd do too — local injection is overkill for partial tears like this. Curious to see how week 4 looks.

Week 2: night waking is down to 2-3 nights instead of 4-5. Pain at rest is maybe 2/10 now. Overhead reach still 6-7/10 if I push it but I don't push it. No side effects at all from the compound.

Nice. Went through something similar on the other side last year and the first meaningful shift for me was around week 3-4. Hang in there.

Week 3: slept through the night 5 nights out of 7. First time since October. Emotionally this is huge tbh. Pain with overhead reach still there but dialed back. Going to try an empty bar front raise next week just to benchmark.

Week 4 check-in: sleep pain essentially gone. Woke once this week and it was because I tried to sleep on it directly (learning curve). Overhead reach pain down to 4/10. ROM has started to open up — measured with a broomstick and I'd say 10 degrees more ER than baseline. Not testing the press. Continuing same dose.

Week 6: reassess time. Overhead reach 2/10 pain. ROM probably 90% of the other side. PT cleared me to start adding light loaded presses with empty bar next week. I'm going to keep BPC going through the press reintroduction and taper off at week 12.

Any thought to adding TB500 for the second half? Not saying you need to but some people stack the two for tendon work.

Honestly I thought about it but the rule of 'change one variable' says no. If this works it works, and I'll know BPC alone did it. If I add TB500 at week 6 I won't know.

Week 8: empty bar press, 3 sets of 8, zero pain. Added 10 lb plates — still zero pain. This is wild. Going to stay conservative on load but this is the first time I've pressed anything in six months without wincing.

Love to read this. Been nursing a knee thing and this thread is convincing me to actually try the protocol instead of just reading about it.

Week 10: up to 95 lb for 3x8. No pain during, no pain after, sleeping fine. Starting to wonder if I'm imagining the injury was ever there. Going to finish the full 12 weeks as planned, no reason to cut short.

FINAL — Week 12 wrap-up:
Starting → ending:
- Rest pain: 3/10 → 0/10
- Overhead reach pain: 7/10 → 0/10
- Night waking: 4-5x/week → 0x/week
- Overhead press: couldn't do empty bar → 115 lb for 3x8, clean
- ROM: missing 20deg ER → within 5deg of other side
Total cost: roughly $220 in compound, $30 in pins/bac water.
Side effects: none. Zero.
Would I do it again: absolutely. If I pull something on the other side I won't waste 8 weeks on PT alone before pulling this out.
Biggest lesson: 500mcg 2x subq is plenty. I was tempted to go higher and glad I didn't. Also, discipline on not loading early is probably as important as the compound itself.
